The incumbent is a social worker whose duties and responsibilities are assigned to the care management of Veterans in the Contract Nursing Home (CNH) Programs. Responsibilities include the management, coordination, and provision of social work services to Veteran and their families in VA contracted CNH facilities. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Examples of creditable experience include but not limited to assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence, advocate with appropriate VA and community service providers and agencies, independently assesses the psychosocial functioning and needs of Veterans and their family members, collaborate with the Veteran, family/caregiver, and interdisciplinary treatment team to facilitate the delivery of health care services, conducts assessment and provides specific interventions, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses, independently formulate, provide a range of interventions and treatment modalities and implement a treatment plan. Resume must include hours per week for work experience credit. Licensure/Certification. Social Worker (Program Coordinator) must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: a, Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring; b. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program; c. Ability to supervise multidisciplinary staff assigned to the program; d. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s); e. Ability to provide training, orientation, and guidance within clinical practice. Utilizing this assessment, the incumbent facilitates the Veteran's maximum use of treatment to attain the highest level of independence possible/practicable and in developing appropriate discharge plans from the CNH to the next appropriate level of care in the community. Actively participates as a member of the interdisciplinary treatment team by collaborating with treatment team members in the mental health clinic, primary care, and inpatient units when Veterans are readmitted to the medical center for stabilization of psychiatric or medical conditions. The incumbent must actively participate through collaboration with Veterans and families and CNH interdisciplinary treatment team members in developing and implementing of a resident centered treatment plan. Facilitates discharge planning through collaboration with Veterans and their families, as well as facility staff and interdisciplinary treatment team members when Veterans in the CNH Program are identified as needing alternative placement or when Veterans' request return to independent community living. Facilitates alternative placement when the CNH facility cannot meet the Veterans' needs. This discharge planning facilitation requires collaboration with Veterans and their families, as well as facility staff and interdisciplinary treatment team members. Liaisons with Veterans, their families, CNH, VA, and community resources to ensure thorough delivery of services (equipment, discharge planning, specialty appointments, customer service, and other needs). Provides treatment modalities such as brief crisis or supportive counseling for Veterans and families, as appropriate. Provides consultation and education to Veterans, their families, and CNH staff regarding community resources, VA benefits and specialty programs, and advance directives. This includes accessing and/or coordinating community-based services, including information and referral for additional services from other VA programs, other government programs, and community programs. Provides consultation to treatment teams and CAVHS staff members as well as CNH staff regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, discharge planning, and ongoing adjustment to the residential care setting. Makes appropriate referrals using the correct process for Veteran's benefits and services, community resources, community agencies/programs and other governmental agencies/programs. Provides Advanced Directive information and education including, but not limited to the following: Durable Power of Attorney for Health Care, Living Will, Health Care Directive, and/or treatment preferences document to Veteran and families. Also assists the Veteran in preparing such documents, as requested. Assists the interdisciplinary team with identifying appropriate decision makers if Veteran is unable to make own decision (Next of Kin, HCPOA, Legal Guardian). Performs appropriate suicide risk screenings and comprehensive suicide risk assessments. Develops and integrates safety planning and safety goals into the Veteran's collaborative, individualized treatment plan. In conjunction with the healthcare team, identifies and assesses risk, safety, supports and implements appropriate interventions/treatment for those who are experiencing abuse, neglect, exploitation or are experiencing domestic or intimate partner violence. Adheres to VA and mandated reporter requirements. Advises and collaborates with interdisciplinary teams throughout the medical center on abuse, neglect, exploitation, and intimate partner violence issues. Integrates Whole Health Principles in assessments and clinical interventions.
